WHO CNS Tumor Grade (2021)
Clinical Context & Background
The 2021 WHO Classification of Tumors of the Central Nervous System classifies tumors into Grades 1 to 4 based on histology and molecular markers. Unlike previous editions, grading is now done within tumor types (e.g., IDH-mutant Astrocytoma is Grade 2-4) rather than across types. Higher grades indicate increased aggressiveness and poorer prognosis.
Formula Logic
Classification based on malignancy, mitoses, necrosis, and molecular features.Reference Data
| Grade | Biological Behavior | Prognosis |
|---|---|---|
| Grade 1 | Low proliferative potential | Excellent (Curative with surgery) |
| Grade 2 | Infiltrative, low mitotic activity | Good (Often recur, can progress) |
| Grade 3 | Histological malignancy | Intermediate (Aggressive, adjuvant tx needed) |
| Grade 4 | Necrosis/Microvascular proliferation | Poor (Very aggressive, rapid evolution) |
