Risk grouping matters because limited-stage Hodgkin lymphoma is highly curable, and treatment is deliberately risk-adapted. A favorable classification may support abbreviated chemotherapy and carefully selected radiation approaches in the right protocol. An unfavorable classification may lead clinicians to consider additional chemotherapy cycles, combined-modality treatment, different radiation fields or doses, or PET-adapted strategies. Clinical caution

This tool is intended for stage I-II classical Hodgkin lymphoma after appropriate staging. It should not be used to classify advanced-stage disease, nodular lymphocyte predominant Hodgkin lymphoma, or cases where pathology or staging remains uncertain.

4. Practical Clinical Workflow

  1. Confirm diagnosis. Ensure classical Hodgkin lymphoma histology and obtain expert hematopathology review when the diagnosis is uncertain.
  2. Complete baseline staging. Use PET-CT/CT, marrow assessment when indicated, labs, and clinical history to confirm stage I-II disease.
  3. Select the classification system before entering data. Choose GHSG, EORTC/LYSA, or NCCN based on the protocol or guideline you plan to follow.
  4. Enter unfavorable factors. Record bulk, nodal areas/sites, ESR, B symptoms, extranodal disease, and age.
  5. Interpret the output alongside PET-adapted treatment strategy. Interim PET response can meaningfully alter subsequent chemotherapy and radiation decisions in many modern pathways.4

5. Interpretation Pitfalls

Unfavorable does not mean advanced.

Early unfavorable means stage I-II disease has risk factors associated with higher relapse risk or different treatment requirements. It is distinct from stage III-IV disease.

The systems can disagree.

A 52-year-old patient may be unfavorable by EORTC/LYSA due to age but not by GHSG if no other risk factors are present. The calculator is designed to make that distinction visible.

PET response is not optional context.

Modern limited-stage Hodgkin lymphoma treatment often incorporates interim PET response. Risk grouping should be combined with response-adapted strategy rather than interpreted in isolation.

6. FAQ

Which early Hodgkin risk system should I use?

Use the system specified by the clinical trial, guideline, or institutional protocol being followed. Avoid mixing cutoffs from different systems in the same treatment decision.

Does favorable risk mean no radiation is needed?

No. Favorable risk is one input into treatment planning. Radiation use depends on protocol, disease sites, PET response, patient age, late-effect risks, and shared decision-making.

Can this calculator be used after relapse?

No. It is intended for baseline limited-stage classification, not relapsed/refractory Hodgkin lymphoma risk assessment.
